In a 3 quart slow cooker, add all ingredients and stir to combine.
Cover and cook on low for 1 hour, or until ingredients are hot and bubbling. Stir once more before serving.

Notes
Oven directions: Combine your ingredients in a bowl, then pour into a 2 quart baking dish. Bake at 350 degrees Fahrenheit for about 20-25 minutes, or until hot and bubbly.
Leftover dip will keep for 3-4 days in a tightly sealed container in the refrigerator.
Rotisserie chicken can be substituted with about 16 ounces of lightly seasoned cooked chicken.
